Important Info About Student-Athlete Physicals for 21-22

Students who did not have their physicals completed at the district event at Magnolia West this past May will need to access the MISD physical packet and get one through a private physician before participating in Athletics this coming year.

It is necessary for students and parents to fill out the official MISD physical packet for 2021-2022 online. These are the ONLY physical forms that will be accepted by the coaches and trainers and should be printed, filled out, and given to your doctor at your appointment.

In addition to a current physical, students are required to register and complete online paperwork in order to be UIL eligible for extra-curricular activities. Magnolia ISD is switching back to Rank One Sport to host its online paperwork for 2021-22. The links to Rank One and directions for creating parent accounts and filling out athletic forms are below:

Once you have set-up a Rank One Sport account, fill out and PRINT a blank copy of the physical and a medical history form. A link to a PDF version of these forms is below. Give the blank copy of the physical to your doctor at your appointment to complete. After all the forms of your physical packet have been completed, they may be dropped off with the principal’s secretary at MJH before the start of the year or can be turned in to your student’s coach during the first two weeks of school.
