Important Info About Student-Athlete Physicals

Students who did not have their physicals completed at the district event at West this past May will need to access the MISD physical packet and get one through a private physician before participating in Athletics this coming year.

It is necessary for students and parents to fill out the official MISD physical packet for 2019-2020 online. These are the ONLY physical forms that will be accepted by the coaches and trainers and should be printed, filled out, and given to your doctor at your appointment.

  • Follow the link to fill out your information, then click “Sign Up” to create new account
 
  • Be sure to “link accounts” between students & parent/guardian with either a cell phone number or email address
 
  • Both of you will each need to log back into arbiterathlete.com and complete the “athletic forms”

Once you have set-up an Arbiter account, fill out and PRINT a blank copy of the physical and a medical history form. Links to a PDF version of each form are below. Give the blank copy of the physical to your doctor at your appointment to complete. After all the forms of your physical packet have been completed, they may be dropped off with the principal’s secretary at MJH before the start of the year or can be turned in to your student’s coach during the first two weeks of school.

Magnolia ISD School Board policy requires a physical annually. This physical needs to be performed on or AFTER April 1st of the year that fall sports begin. Physicals performed in the fall of the year before will not be accepted.

The forms need to be completed on the current physical forms and all previous forms will not be accepted. The University Scholastic League (UIL) periodically changes the wording or either adds or deletes certain portions of the required paperwork, so it is VERY important to use the current forms while completed required paperwork. If you are not certain as to the form you have, you can always call the athletic trainers at West to verify you have the correct form.  Again, if you have any questions, please call the athletic training office at 281.252.2550 ext. 7266.
